Please be prepared to provide some personal information. New government regulations require us to ask for your social security number, birthdate, primary and residential address, mother’s maiden name, and a copy of your driver’s license, as well as state- or government-issued ID.

CHANGE YOUR
DIRECT DEPOSITS
Send one completed and signed Direct Deposit form to each company or organization and automatically deposits it into your existing account. OR … bring us the list of your direct deposit information, and we’ll help you set it up.
Note: If your direct deposit involves Social Security, V.A. Comp or Pension, Supplemental Security Income, Railroad Retirement, or Civil Service Retirement, no form is needed, and an Adams Community Bank Representative can set this up for you!

CHANGE YOUR
Automatic payments
If any payments are automatically paid out of your account, send one completed and signed Automatic Payment Form to each company or organization debiting from your existing account. OR … bring us the information, and we’ll help you set it up.

Stop Using Your
Old Checking Account
Destroy all unused checks, deposit slips, ATM, and Debit cards. Ensure all your checks have cleared and automatic payments and deposits have switched before closing your old account. Use the Account Closing Request Form to notify your former bank of closing your account.
KEEP TRACK OF YOUR SWITCH
Use this form to track all the information you need to move your direct deposits and automatic payments. If you choose to have your personal banker take care of your switch, you will be informed of the progress.
